Officials feedback form

This form is meant to supply direct feedback to the LOBHL Executive members on the league officials. If there is negative feedback, we ask that you exercise the 24 hour rule before submitting your feedback. We also hope that when an official does an excellent job, we also pat them on the back for that as well! We will use this feedback tool to reach out to the officials and work with them to improve our league!
